  • Hazeltons: Community based screening (location varies depending on families), every 4-6 weeks on a Tuesday or Wednesday.
Infant Hearing Screening Less than 6 months old infants. Hearing Screener from Smithers will contact family. Contact information is provided by the birthing hospital through the B.C. Early Hearing Program's database. Or for home based births and babies born of the Province of B.C., Public Health then provides the information to the BC Early Hearing Program.

Tier Two Screening – (Bulkley Valley and Hazeltons)

Community Living of BC Community Living of British Columbia (CLBC) 1.  Developmental Disability- an assessment completed by an approved professional (certified school psychologist) that clearly states they meet 
the “Diagnostic Criteria for Mental Retardation” cited in the DSM-IV-TR.
2.  Personal Supports Initiative-
          1. an assessment indicating they do not meet the “Diagnostic Criteria for Mental Retardation” cited in the DSM-IV-TR; AND
           2. an Adaptive Functioning report completed by an approved professional that says the person has significant limitations in  adaptive functioning. CLBC will accept one of three assessment tools for the Adaptive Functioning report. The assessment  report must state which assessment tool was used; AND
  3. Either
                 a. Assessments confirming the diagnosis of Fetal Alcohol Spectrum Disorder (FASD) completed by an approved  professional, OR
                b. Assessments confirming the diagnosis of Autism Spectrum Disorder (ASD) completed by an approved professional.
 
Age: 19 year old and +
1.  Developmental Disability
     An adult has:
     ·Significantly impaired intellectual functioning
     ·Significantly impaired adaptive functioning 
     ·These limitations must have started before age 18.
2. Personalized Supports Initiative
    An adult who does not have a developmental disability but has:
     ·Significant limitation in adaptive functioning, 
     AND EITHER:
     ·a diagnosis of Fetal Alcohol Spectrum Disorder (FASD) OR
     ·a diagnosis of a Autism Spectrum Disorder (ASD).

To be noted: A qualifying practitioner means either a Psychologist or a Psychological 
Associate registered with the College of Psychologists of British Columbia or a Certified 
School Psychologist certified by the British Columbia Association of School Psychologists.
